What should my documentation include?

To register for disability-related academic accommodations with SAS, you’ll need to provide documentation to help us understand your needs and determine the appropriate support. All documentation must be in either English or French.

Documentation Requirements

Your documentation should confirm that you have a diagnosed disability and explain how it affects your academic participation.

Required information:

  • Temporary or permanent condition
  • Symptoms or barriers experienced (e.g. chronic pain, difficulty concentrating, note-taking challenges)
  • Level of academic impact (e.g. mild, moderate or severe)

Diagnosis is not required. You do not need to disclose your specific diagnosis.

Ready to register? Submit your documents.

What documents can I submit?

The type of documentation you will need to submit depends on the type of disability you have. Our medical form allows your health care practitioner to share information about multiple disability related impacts that you are experiencing.

If you have a learning disability as well as other disabilities you can submit both a psychoeducational assessment and our medical form.

Learning disabilities

  • A Psychoeducational Assessment is required if you have been diagnosed with a learning disability/disorder.

This assessment should have been completed within the last five years, unless you were at least 18 years old when you were tested. We will accept older assessments as a starting point to register with SAS.

For more information about psychoeducational assessments and what the psychologist should include in a psychoeducational assessment view and share the Psychoeducational Assessment Practitioners Checklist.

Frequently Asked Questions

You can still register with us, however, access to academic accommodations (such as accommodated tests and exams) might be limited and/or unavailable this term.

We can explore interim academic accommodations while your assessment is being completed. Your family doctor (or specialist) can indicate on your SAS Medical Form that you are in the process of being assessed. They are asked to provide information relating to your functional limitations on the form.

Follow the three steps to register with us.
